Registration of Death

To register death of a Croatian citizen in the Croatian Registry the following documentation should be provided:

• Original Death Certificate from the respective registry office in Australia/New Zealand with Apostille seal and certified translation in Croatian 
• proof of Croatian citizenship for the deceased as well as an identification document of the deceased (if the deceased had a Croatian passport or ID Card, they should also be submitted with the application for cancellation).
• Valid photo-identification of the person requesting the registration of death
• Filled in form - prijava smrti

Please note that registration of death is possible only upon completion of all prior registrations related to changes of status or name of the deceased (eg. marriage, divorce, name change etc.).
 
Applicants should make sure that the personal information of the deceased matches the information in the Croatian Registry books (name, surname, date and place of birth, parents’ information, marital status).
