Application for Schengen Visa (visa A and visa C)
Applicants must fill out the prescribed visa application form in Latin alphabet, with capital letters and sign it personally.
A third-country national who is the beneficiary of the applicant's travel document must also fill out the prescribed visa application form.
The application for a minor national of a third country or a national of a third country deprived of legal capacity is submitted and signed by his legal representative.

Application for a long-term visa (visa D)
Application for a long-term visa (visa D) shall be submitted on the form prescribed by Annex 1.B of the Ordinance on Visas of the Republic of Croatia (Official Gazette No. 109/21, effective as of 1 November 2021).
Applicants must fill out the visa application form in Latin alphabet, with capital letters and sign it personally.
Any alien beneficiaries of the applicant´s travel document must also fill out the visa application form.
A visa application for an alien minor or an alien deprived of their business capacity must be submitted by their legal representative, who must sign the form.
